The Duane & Isabelle Schultz Milan Area Schools Alumni Scholarship Fund guarantees an annual income to be used for granting scholarships to college-bound Milan High School seniors. Since only the earnings may be spent, the fund itself will endlessly generate interest and dividends over the years. To contribute, please specify “Alumni Scholarship Fund” on this form and mail it with your contribution.

Pre-Qualification Questionnaire

Please answer the following questions about the program to be funded:

YesNo	Is it to serve the greater Milan area residents?
YesNo	Does it avoid duplication of services that can better be given by existing agencies?
YesNo	Will the program have an obviously beneficial impact on the greater Milan area community’s quality of life?
YesNo	Does the program operate without United Way support?
YesNo	Can the sponsoring group applying for funds assure the continuance of the program to a worthwhile conclusion or a stable ongoing status?
YesNo	Is government funding insufficient to ensure the project’s implementation, even though the project is important enough to warrant a grant?
YesNo	Does the project benefit the community without regard to religious faith?
YesNo	Is the program completely divorced from politics?

Notwithstanding these guidelines, the Greater Milan Area Community Foundation reserves the right to accept, modify or reject any grant request. At minimum, the program to be funded should be able to give an affirmative answer to the above questions.
